Testosterone replacement therapy (TRT) has helped hundreds of thousands of men to reclaim their energy, libido, mental acuity, and physical vitality. But there’s a piece of the testosterone replacement puzzle that often gets overlooked—or outright ignored—especially by providers who lack expertise in hormone optimization (or those who cut corners).
We’re talking about estrogen management.
Many men begin TRT expecting to feel great once on treatment, and instead find themselves experiencing water retention, mood swings, low sex drive, erectile dysfunction, and fatigue (with some of these being the same challenges TRT is meant to solve). In many cases, the culprit isn’t an issue with testosterone levels continuing to be suboptimal. Rather, the problem is unchecked estrogen.
Some of the most important aspects of best practice when it comes to properly prescribing TRT for men are:
- Understanding why estrogen levels rise while men are on TRT.
- What happens when estrogen becomes elevated.
- What can be done to manage estrogen levels so results from TRT can be maximized.
What Happens to Estrogen Once TRT Is Started?
When exogenous testosterone is introduced into a man’s body, it isn’t just testosterone levels that will go up. That additional testosterone also gives the body more raw material to convert into estrogen.
This conversion happens through a process called aromatization—a biochemical reaction carried out by an enzyme called aromatase. Aromatase is found throughout the body, with the highest concentrations located within fatty tissue (particularly around the midsection). It converts testosterone into estrogen.
In men with adequate endogenous testosterone levels, this conversion of testosterone to estrogen happens at a relatively modest rate. But when testosterone levels rise significantly—as they do when men are on TRT—more testosterone becomes available for aromatization, and estrogen levels climb (sometimes substantially). Men with higher body fat percentages tend to experience this more acutely than those with low body fat, because they have more aromatase-rich tissue available.
Without an estrogen management strategy in place, most men on TRT will end up with testosterone levels that are optimal, but an estrogen level that creates a whole new set of problems.
What Elevated Estrogen Looks Like in Men
Contrary to what many people believe, estrogen isn’t strictly a “female hormone”. Men need estrogen too (and we’ll get to more on that in a minute). But when estrogen climbs too much, the symptoms can be significant and also confusing (because some of them overlap with the symptoms of low testosterone that were a problem in the first place).
Common symptoms of elevated estrogen in men include:
Emotional and psychological symptoms—Irritability, mood swings, anxiety, and emotional sensitivity that feels out of character.
Sexual dysfunction—High estrogen in men frequently suppresses libido and can cause or exacerbate erectile dysfunction (even when testosterone levels are perfectly optimized). If a man on TRT is still struggling with low drive or poor erections, estrogen is one of the first things that should be investigated.
Water retention and bloating—Estrogen causes the body to hold onto water. Men on TRT without estrogen management often notice puffiness around the face, midsection, or ankles, as well as a generally “soft” look, even when they’re training regularly.
Gynecomastia—This refers to the development of breast tissue in men—sometimes called “man boobs.” Elevated estrogen can stimulate the growth of breast gland tissue. Once it develops, it doesn’t always reverse on its own. This makes prevention of gynecomastia (and therefore management of estrogen conversion) essential.
Fatigue and brain fog—Despite having healthy testosterone levels, men with high estrogen often feel tired, unfocused, and mentally sluggish. This hormonal imbalance can blunt many of the cognitive benefits TRT is supposed to deliver.
Inflammation—Elevated estrogen has been associated with increased inflammatory activity in some men, particularly in the prostate.
The Role of Estrogen Blockers (aka Aromatase Inhibitors)
The most common tool for managing estrogen while men are on TRT is a class of medications called aromatase inhibitors (AIs). These medications work by blocking the aromatase enzyme, thereby reducing the conversion of testosterone to estrogen.
The right medication, dose, and frequency will vary significantly from one man to another. Body fat percentage, baseline estrogen level, testosterone dose, injection frequency, and individual response to aromatase all influence what protocol will make the most sense.
You Don’t Want Estrogen Too Low, Either
Here’s where many men and even some providers get into trouble: estrogen is not the enemy. In men, estrogen is important for the following:
- Bone health—Estrogen is essential for maintaining bone mineral density in men. Low estrogen is a significant risk factor for developing osteoporosis.
- Cardiovascular function—Estrogen supports healthy cholesterol profiles in men and has protective effects on the cardiovascular system.
- Joint health—Estrogen has anti-inflammatory properties and helps to maintain cartilage and joint function. Men with low estrogen commonly struggle with joint pain.
- Sexual function—While elevated estrogen can suppress libido and cause erectile dysfunction, estrogen that is too low can cause the same problems.
- Mood and cognition—Men with chronically suppressed estrogen often experience depression, irritability, and poor mental acuity—essentially the same symptoms as when estrogen is too high.
- Energy—While fatigue is a symptom commonly associated with estrogen that it too high, energy levels can also be compromised by estrogen that is too low.
These last three point to the importance of estrogen optimization. Estrogen needs to be neither too high now too low to avoid these issues.
This is why blanket AI use without lab monitoring is a mistake. A man who takes too much of an aromatase inhibitor will crash his estrogen into a range that will make him feel terrible—possibly even worse than before he started TRT. Because the symptoms of high and low estrogen overlap so heavily, experienced physician oversight is essential.
The bottom line is this: the goal isn’t to eliminate estrogen. It’s to keep it within a healthy, functional range.
Why Estrogen Management Is Often Overlooked
There are a few reasons estrogen management is sometimes left out of TRT protocols:
Outdated or incomplete prescribing—Many physicians who prescribe TRT are not deeply experienced in hormone optimization. They will often monitor testosterone levels without routinely checking estrogen.
Cutting corners and costs—AIs require an additional prescription and additional lab monitoring.
The “wait and see” approach—Some providers like to see if symptoms of elevated estrogen develop before adding an AI into TRT regimens. While this makes some sense in theory, it can mean weeks or months of a patient feeling terrible while estrogen creeps up. An experienced provider will pre-emptively control estrogen conversion upon commencement of testosterone replacement.
Overcorrection in the other direction—In an overreaction to the overprescribing of AIs (which does happen), some providers have swung too far in the other direction (to recommending no AI use at all). However, this can be equally problematic.
Trying to reduce estrogen conversion with something other than an AI—Some providers will try to use the supplement “DIM” to reduce estrogen conversion. While DIM is beneficial for other reasons (that are beyond the scope of the article), reducing estrogen conversion is not one of them. The only way to meaningfully manage estrogen conversion when testosterone levels are optimized via TRT is with a properly dosed AI.
The solution to all of the above is individualized care. Regular lab monitoring and experienced physician oversight are non-negotiable for anyone on TRT who wants to feel their best and not suffer from side effects.
Questions to Ask If You’re Already on TRT
If you’re already on TRT and haven’t discussed estrogen management with your provider, following are some questions to ask:
- Is my estrogen level being tested at regular intervals? (It should be.)
- What is my current estrogen level?
- What is considered an optimal estrogen level for men?
- Am I a candidate for an aromatase inhibitor based on my response to therapy?
- What symptoms should I watch for that might indicate an estrogen imbalance?

The Bottom Line
Testosterone replacement therapy can be life-changing for men with low testosterone—but getting the most out of it requires more than just optimizing testosterone. Estrogen management via an appropriately dosed aromatase inhibitor (guided by regular lab work) is a critical component to a well-designed TRT protocol.
Meanwhile, unchecked estrogen undermines many of TRT’s core benefits, and can create a frustrating cycle where a man feels only marginally better—or even worse—despite having optimal testosterone numbers. With proper monitoring and individualized management, these issues are entirely preventable.
