GLP-1 medications (like semaglutide and tirzepatide) have resulted in weight loss finally being an attainable goal for those struggling to lose weight. And the results can be dramatic, with many patients losing significant amounts of weight in a relatively short period of time.

But there’s an important part of this picture that doesn’t always make it into the headlines. Not all of the weight that is lost is necessarily fat.

Studies have found that a significant portion of total weight lost while on GLP-1 medications can come from lean tissue, rather than fat.

This matters, because not all weight loss is good weight loss. Losing fat improves metabolic health. Losing muscle, on the other hand, does the opposite.

Unfortunately, rapid weight loss of any kind (whether it’s from GLP-1 medications, aggressive dieting, or bariatric surgery) tends to come with a higher proportion of muscle loss, as compared to slower and more gradual weight loss.

Why Muscle Loss Happens During Rapid Weight Loss

A few factors that can accelerate muscle loss during GLP-1 therapy include:

  • Reduced appetite. GLP-1 medications work in part by suppressing appetite. While this supports weight loss, it also makes it easy to fall short on protein intake, a nutrient essential for maintaining muscle mass.
  • Caloric deficit. Any significant calorie deficit can result in muscle tissue being broken down for energy if the body isn’t given adequate protein intake and resistance training stimulus to protect it.
  • Reduced activity. When people lose weight quickly, they can become less physically active as a consequence of fatigue.
  • Age-related muscle loss. For adults over the age of 40, some degree of muscle loss (known as sarcopenia) is often underway. Rapid weight loss can accelerate this process if it isn’t actively counteracted.

Losing muscle mass isn’t just a cosmetic concern. Muscle plays a central role in metabolism, blood sugar regulation, strength, and mobility. Muscle loss can lead to:

  • A slower resting metabolism, which can make weight maintenance more difficult.

The good news is that muscle loss during GLP-1 therapy is largely preventable, with the right approach:

  1. Prioritize protein intake. Because appetite is often suppressed while on GLP-1 therapy, it takes intentional effort to make sure protein remains a priority at each meal. Lean meats, eggs, dairy, and legumes are all good options.
  2. Incorporate resistance training. Strength training sends a direct signal to the body to preserve, rather than break down, muscle tissue. Aim for regular resistance exercise several times per week.
  3. Consider supportive supplementation. Creatine, in particular, is a well-studied supplement that can help to preserve muscle strength and mass during periods of caloric restriction. Amino acids like glutamine, arginine, and carnitine can also be helpful.
  4. Optimize your hormones. Testosterone and growth hormone both play a significant role in maintaining lean muscle mass. Optimizing these hormones alongside GLP-1 therapy can help to protect against muscle loss.
  5. Monitor body composition, not just the scale. Tracking changes in muscle mass and fat mass, rather than relying on weight alone, provides a much clearer picture of whether a weight loss strategy is working the way it should.
