  • Gynecomastia (i.e. “man boobs”)
  • Nipples that are itchy, swollen, or sore
  • Weight gain (especially in the form of belly fat)
  • Water retention

  • Depression
  • Prostate issues

Importantly, men who receive testosterone replacement therapy that does not include estrogen management will likely experience accelerated estrogen conversion, particularly if they have higher body fat percentages or metabolic syndrome.

Estrogen Deficiency

Conversely, overly aggressive estrogen suppression can lead to its own set of symptoms as a consequence of estrogen that is too low, including:

  • Joint pain
  • Low libido
  • Erectile dysfunction
  • Depression

In addition to the symptoms noted above, men with estrogen levels that are too low for too long are more likely to suffer when it comes to their cardiovascular, bone, and brain health.

Comprehensive Lab Testing

Prescribing TRT correctly means checking more than just testosterone levels when ordering lab work. Estrogen should be checked, too.

Beyond that, the interpretation of estrogen values requires nuanced clinical expertise that will take into consideration more than the lab’s reference ranges. And yet, where that small window lies will vary from one man to the next.
