If you had to pick one metric to sum up how well your body is aging, what would it be?

Blood pressure? Cholesterol? Body weight?

All of these are important. But there’s another number that outperforms each of these when it comes to predicting longevity. And it’s a number most people have never measured.

That number is called VO2 max.

What Is VO2 Max?

VO2 max is a measure of how much oxygen your body can use during intense exercise.

Think of it as your body’s engine size. Oxygen has to travel a long road to reach a working muscle. It enters through your lungs, gets loaded onto red blood cells, is pumped by your heart through your blood vessels, until it finally reaches the mitochondria inside your muscle cells where it’s converted to energy.

VO2 max measures how efficiently that entire chain of events works together.

Which is why it’s such a powerful indicator of overall health. This one metric measures the combined condition of your lungs, heart, blood vessels, muscles, and mitochondria…all in one single number.

Very few tests tell you that much all at once.

Why It Matters So Much

Here’s where it gets interesting.

Large studies following tens of thousands of people over many years have consistently found that low cardiorespiratory fitness is associated with a higher risk of death from all causes. And the size of that association is striking in that it’s comparable to or greater than well-known risk factors like smoking, diabetes, and high blood pressure.

Even more encouraging is what researchers have found at the other end of the spectrum:

There doesn’t appear to be a point where more fitness stops helping. People in the highest fitness categories consistently show the lowest mortality risk.

The takeaway? Improving your fitness isn’t just about looking better or performing better. It’s one of the most meaningful things you can do to promote longevity.

VO2 Max Declines with Age

Starting around age 30, most people will lose about 10% of their VO2 max per decade. And that rate of decline tends to accelerate after the age of 60.

This is why activities that once felt easy, start to feel hard. Carrying groceries up a flight of stairs. Keeping up on a hike. Playing with your dog. Doing yardwork. The list goes on and on.

There’s a threshold worth knowing about here. Researchers have identified a level of aerobic capacity below which independent living becomes difficult. The point at which ordinary daily tasks require near-maximal effort. Someone who starts with a low VO2 max at 50, and who declines at an average rate, may cross that threshold decades earlier than someone who started with a higher number.

In other words, your fitness today could be an investment in your future independence.

How Is VO2 Max Measured?

The gold standard for testing VO2 max is a graded exercise test performed in a lab, where you exercise to exhaustion while wearing a mask that measures the oxygen you consume, and the carbon dioxide you exhale.

While this type of test won’t be accessible to everyone, there are reasonable alternatives:

  • Many fitness watches and trackers now provide VO2 max estimates based on heart rate and pace data. They’re not lab-precise, but they’re useful for tracking your own trends over time.
  • Submaximal field tests, like a timed one-mile walk or a 12-minute run, can produce solid estimates.
  • Many fitness centers offer some version of VO2 max testing.

How to Improve VO2 Max

The good news is that VO2 max responds to training at any age. Studies in adults well into their 70s and beyond have achieved meaningful improvements.

The most effective approach combines two types of work:

  • Lots of easy aerobic activity. Walking, cycling, swimming—at a pace where you can still hold a conversation. This kind of exercise should make up the majority of your training.
  • A small amount of hard interval work. One or two sessions per week of short, intense efforts that are followed by recovery. This is what drives the biggest gains in maximum capacity.
  • Strength training belongs in the mix too. Muscle is where oxygen is ultimately used.

The Hormone Connection

Here’s the part that often gets overlooked:

Your ability to build and maintain aerobic capacity depends on your hormones:

  • Testosterone supports the muscle mass and red blood cell production that oxygen delivery depends upon.
  • Thyroid hormones regulate the metabolic rate at which your cells consume that oxygen.
  • Growth hormone drives tissue repair that lets you recover between hard workouts.
  • Estrogen supports blood vessel flexibility that keeps circulation efficient.

When these hormones decline with age, your VO2 max ceiling drops, no matter how disciplined your training is. Many people work harder and harder, while their fitness stubbornly refuses to budge, never realizing that the problem isn’t effort. It’s physiology.
